North Fork Breast Health Coalition "Lend A Helping Hand" Grant
If you have been diagnosed with breast cancer and are currently under a physician’s care, you are eligible for a one-time grant of up to $1,000, which may be used for such services as house cleaning, meal preparation, transportation expenses, or just a day of relaxation at a local spa. Who is eligible?
- Individuals who have been diagnosed with breast cancer and are undergoing chemotherapy or radiation therapy and can provide confirmation by their physicians.
- Individuals should live on the North Fork or are currently under treatment by a physician on the North Fork.
- Individuals must sign a consent and release form.
